​​​​​The Pennsylvania Patient Safety Advisory was the official publication of the Patient Safety Authority from 2004 to 2019, succeeded by the current journal, Patient Safety. The Advisory featured several in-depth, first-of-its-kind analyses based primarily on reports from the Pennsylvania Patient Safety Reporting System (PA-PSRS).​​​
