Small Miscellaneous Items (SMIs) and Unretrieved Device Fragments (UDFs) are the most commonly retained surgical items. Needles are the most frequently miscounted items yet retention is uncommonly reported. Guidewires are the most commonly retained Devices. What’s going on here? In light of the frequency with which these events occur and the spectrum of places these events occur in, OR and hospital personnel need to look at preventive strategies and harm mitigation efforts. Is the risk of retention really less than the risk of retrieval? If indeed retention does occur then understanding disclosure and reporting responsibilities are necessary.
