Falls Prevention: Pennsylvania Hospitals Implementing Best Practices
Pennsylvania Patient Safety Authority analysts evaluated survey responses from hospitals participating in the falls prevention collaboration and identified 35 practices or program elements
that were associated with lower rates of falls with injury. Assessing best practices, auditing for compliance, and analyzing results in relation to rates of falls with injury can help organizations effectively focus their improvement efforts.
Breakdowns in the Medication Reconciliation Process
Breakdowns in medication reconciliation during admission, transfer, or discharge introduce potential risk to the patient. Over one year, 501 events involving these breakdowns were reported to the Authority. Implementing strategies to limit these errors and ensure accurate medication histories can help foster safe transitions from one level of care to the next.
